History
History of present illness:
After taking out my locks, I started straightening my hair with hear and using a curling iron. My scalp started itching all the time. I stopped using heat and went natural. after a couple of weeks the itching stopped. I now have bald spots throughout my scalp. It looks like the hair is trying to grow back though.

Medical History:
HTN, seasonal allergies

Past surgical history:
none

Allergies:
seasonal allergies, nickel

Medications:
Bystolic 10mg daily. HCTZ 12.5 daily

Family history:
COPD, Breast Cancer, Prostate cancer

Social history:
married, with two adult children. Quit smoking 1ppd ten years ago. Run at least 3 times per week. Work 40 hours/week.

previously itchy; now balding scalp
