fever for 5days
cough for 5 days
streaky hemoptysis for 4 days
breathlessness for 4 days

no PTB
no major medical illnesses
no history suggestive of immunocompromised state

Past surgical history:
appendicectomy done 2 years back

Allergies:
not known

Medications:
took paracetamol for fever

Family history:
no similar illnesses

Social history:
low SES

General:
pallor+
B/L 1cm. axillary lymphnode+

Temperature:
102degree farenheight

Heart rate:
156/m

Blood pressure:
100/60mmHg.

Respiratory rate:
44/m

Oxygen saturation:
84% on 100% oxygen

Head, ears, eyes, nose & throat:
Left eye- Subconjunctival hemorrhage

Neck:
normal

Chest:
B/l crepts+

Cardiovascular:
Normal

Abdomen:
Normal

GU:
Normal

Rectal:
Normal

Neurological:
No focal neurological deficit

Skin:
petechial haemorrhages over anterior abdominal wall

Laboratories:
Hb-8gm%, TC- 1050/cmm,DC-n-70%,l-30%
platelets-54000/cmm,PT/APTT-deranged
LFT- ALT raised to 134U/l,RBS/UREA/CREATININE-normal

Radiology:
miliary shadows B/l
